Resources for Authorized Signatories

Authorized Signatory: You may be authorized to sign, for your unit, one or more of the types of forms listed below. These authorizations are updated with the Graduate College each year by the Executive Officer of your unit using the Graduate College Roles & Access Manager. Please note that these assignments each span the entire academic year, August to July.

An Authorized Signatory is named by the Executive Officer of a unit using the GC Roles and Access Manager.

NOTE: In most cases, if you have signed a form already as the advisor, another authorized signatory must sign as the department signatory.

Re-Entry

Domestic students who have not been registered for three consecutive terms or international students who did not register for a Fall or Spring term who are either applying to a new program or re-entering a program they took a leave from, need to apply for re-entry.

Petitions for Exceptions to Graduate College Policy

Individuals in units can be authorized to sign petitions as a representative of a unit and indicate and explain to the Graduate College whether the unit supports the request or not.

In this role please realize that petitions are requests for exceptions to Graduate College Policy and must be justified. Departmental rules may be more strict or prohibitive than Graduate College rules, and exceptions to departmental rules do not require a Graduate College petition (but should be recorded in the student’s departmental file). Before commenting on a petition, please ensure that the student’s adviser has commented and signed.

Records

  • Transfer of Credit
    Signing this form is approval of:
    • credit taken as an undergraduate or professional student at UIUC, or
    • from another graduate degree program at UIUC, or
    • as a non-degree student at UIUC or
    • from another accredited institution

    to apply toward the current graduate degree program.
     

  • Curriculum Change
    Curriculum changes include addition or removal of a graduate minor or a graduate concentration from the student record or change in major or degree to a new program of study.
     
  • Change of Level
    Approval of the request to transfer graduate level coursework taken while an undergraduate, but not used toward an undergraduate degree or taken while enrolled as a Professional student (Veterinary Medicine or College of Law JD program) toward the current graduate degree program.

Registration

Doctoral Examination Committee Requests

Individuals in units can be authorized to submit requests for the appointment of doctoral preliminary and final examination committees for students in the unit via the online committee request system. Please note that this role is very important throughout the year, especially near the end of each of the three academic terms.

Thesis/Dissertation Approval form (TDA)

Individuals in units can be authorized to sign the TDA form, verifying that the signatures of the committee are genuine, that a thesis or dissertation meets with departmental approval and may be deposited in the Graduate College. This role is different from that of thesis format reviewer. Please note that this role is very important throughout the year, especially near the end of each of the three academic terms.

Doctoral Exam Result (PER/FER) forms

The Preliminary Exam Result (PER) form and the Final Exam Result (FER) form verify that the student has completed the examination, regardless of the outcome. These forms were formerly called the Certificate of Result. The exam result form is signed by the committee at the conclusion of the examination, and electronically reproduced (e.g., via fax or scan) signatures will be accepted from committee members. Original (wet) signatures are required from the committee chair(s) and the department head, or a designated authorized signatory. The Department Head (or authorized signatory) verifies that the exam and the result are acceptable to the department and that the committee signatures are authentic.

Rating Forms for Fellowships and Traineeships

Individuals in units can be authorized to sign the rating forms used by the Graduate College Fellowship Office to nominate students and to process fellowship and traineeship appointments.
